Just saw this last night. What a great movie. I think there are some huge plot holes if looked at logically - some of them are mentioned here. But the fact that the murder is presaged in his dreams and it matches the "reality" EXACTLY says to me that it's all playing out in his perception. I think that's very intentional. I think he's an unreliable narrator, and that's part of his disassociation from his perception of himself professionally. It's not just conscience or his skills failing or the fact that he's been outplayed by his rivals (or as mentioned above, his own carelessness). It's all of it.
